Location: On site (Head Office based in Athlone, but also visiting clients around Co. Roscommon, Westmeath and Longford)
Employment Type: Part-Time (Monday - Friday)
Job Description:
As a Registered Nurse - Care Planning Specialist, you will be responsible for creating, implementing, and monitoring personalized care plans for our clients. You will collaborate with a multidisciplinary team to ensure our clients receive the best possible care tailored to their individual needs.

Key Responsibilities:
- Develop and manage comprehensive care plans for clients based on assessments and individual needs.
- Coordinate with healthcare professionals, clients, and families to ensure cohesive and effective care.
- Monitor and evaluate client progress, making necessary adjustments to care plans.
- Provide education and support to clients and their families regarding care plans and health maintenance.
- Document all care planning activities and maintain accurate client records.
- Participate in regular team meetings
**Qualifications:**
- Current and valid Registered Nurse (RN/RNID).
- Minimum of 2 years of nursing experience, preferably in care planning or a related field.
- Strong assessment, critical thinking, and organizational skills.
- Excellent communication and interpersonal abilities.
- Proficient in using electronic health records and other office software.
- Ability to work independently and as part of a team.
- Full Driving License

Experience:
- Nursing: 3 years (required)
- Care planning: 1 year (preferred)
Licence/Certification:
- Driving Licence (required)
- Nursing and Midwifery Board of Ireland registration (required)
